Selenium 3/TestNG giving N/A test result - java

I've recently upgraded to Selenium 3.141.59 from 2.47.0. Since the upgrade, our test suites have been receiving random N/A test results. It happens to about 1% of the tests and I have not been able to track down any meaningful logs to determine the cause.
We're using Selenium Grid Extras as our distribution.
Here is what a report looks like:
I am able to see that the test is being sent to the node by the hub:

I am at a complete loss and have not been able to find other with the N/A test result. Any help would be appreciated.

Derby client jdbc driver connection error : Failed to create database 'sample'

I started the Derby Network Server like stated in this tutorial - here.
But when i typed:
ij> connect 'jdbc:derby://localhost:1527/sample;create=true',
I get the following error:

The log entry states:
Directory C:\Program Files\Java\jdk1.8.0_45\db\bin\sample cannot be created
So as far as I know this could be some permission issue with network server process not having enough rights to create a directory in C:\Program Files\Java\jdk1.8.0_45\db\bin\. You can try to run it as administrator.
From your exception stack trace it looks like your Derby network server process was started in the folder C:\Program Files\Java\jdk1.8.0_45\db\bin\ and the process does not seem to have the privileges to write the (Derby database) files in that folder. Try to find a folder where the Derby server process has the privilege to write files into. You can give specify windows drive letters and paths in the Derby connect URL. Just make sure to convert any back slashes \ to forward slashes / or escape them like this \\. So if for example if you know the Derby server process can write to d:\temp folder, then you can compose your command as
ij> connect jdbc:derby://localhost:1527/d:/temp/sample;create=true
or
ij> connect jdbc:derby://localhost:1527/d:\\temp\\sample;create=true
Beware substituted drives when using Derby - if you put your database on a substituted drive it won't work and you get confusing nio exceptions and class cast errors in the log. So this does not work:
connect 'jdbc:derby://localhost:1527/t:/DerbyDB;create=true';
where t is substituted. The same command pointed at drive c does work:
connect 'jdbc:derby://localhost:1527/c:/DerbyDB;create=true';
There is a discussion of these issues on the Derby site, - but maybe its not fixed for all cases? including where the database itself is on a substituted drive. I'm currently using version 10.10.2.0

If i run the code by keeping debug point, then able to see the response but it is not binding to SoapUI.
I even tried by seting Socket Timeout = 0.

Can't understand why the connection is refused.
I uses Windows 7 x64 , ECM Documentum 6.7 for , IE 8(for 32 and 64)/ IE9 (for 32 and 64), java 1.6. Is anybody can help me?

Application upgrade throws NPE

I am using JarDiff+Pack200+Signed app/jars
I have application v1.0 that requires j2se version=1.6.0_16
After the 1st installation/download I deploy v2.0 that requires j2se version=1.7+
The upgrade fails on random jars with the Null Pointer Exception
The second attempt to start v2 is always successful.
I have tested it from command line with java 1.6.0_16 and 7u7.
Any ideas what it means?

java.io.IOException: An existing connection was forcibly closed by the remote host

I have a server running Apache FTPServer. The FTP server is functioning well until recently some upload from the client started to fail with this error that is attached. I have increased the MaxIdleTimeout but is still encountering this error. Anybody has any idea to what actually generated this error?

It looks like what the error says, the server closed your connection. Typically this is as a result of inactivity on your end. If you don't keep talking to the server it will close your connection. If the connections idle you need to send a stream of NOOP commands to it to let it know your still there.
